YOU MUST SUBMIT PROOF OF UNIT AFFILIATION via file upload. Please read all instructions before applying.

In order to become an AWCT customer, you will need to complete and submit the online form below and provide one of the following for proof of your unit affiliation:

• Letter stating your position within your performance unit on school letterhead, with the signature of the principal or athletic director
• Proof of the band booster organization such as voided check or bank deposit slip with band booster name and address on the form (School accounts are paid by the school via PO, check or credit card; band booster accounts are paid by band boosters' check or credit card.)
